Participation Forms

O’Dea High School participation forms can be accessed through Magnus Health SMR (Student Medical Record) or “Magnus”, an online database.  As this is a web-based system, you will have continuous access to your student’s information stored there as well as the ability to make updates when needed.

Each student-athlete must complete these forms in order to participate in sports at O’Dea High School. All participation forms must be completed by the first practice. 

You can access your Magnus account through your PowerSchool Parent Portal. The application button is in the upper right tool bar and is titled “Magnus Health.”
