Planned Absence Form

Athletic forms are filled out on Magnus Health. As this is a web-based system, you will have continuous access to your student’s information stored there as well as the ability to make updates when needed.

Each student-athlete must complete their forms in order to participate in sports at O’Dea High School. All participation forms need to be completed prior to the first practice.

You can access your Magnus account through Veracross.
