Participation Forms

Each student-athlete must complete the required participation forms by the first practice of the season to participate their chosen sport. These forms can be accessed through Magnus Health, an online database.

You can access your Magnus account through Veracross. The application button is in the upper right tool bar and is titled “Magnus Health.”
